Inlay and onlay fillings are divided into two categories based on the materials used in their contents:
Porcelain inlay/onlay fillings: Similar to aesthetic porcelain crowns, this type of fillings is used to anatomically complete the missing tooth parts with porcelain.
Composite inlay/onlay fillings: Recently the composite fillings prepared in clinics for posterior teeth have given extremely satisfactory results. For this reason, inlay-onlay composite fillings are not preferred much anymore.
♦ Molar teeth with excessive material loss and/or one or two fractures
♦ Molar teeth with problems of interdental decay on both side
♦ Molar teeth with excessively thin dental tissues and more fragile due to root canal treatment
♦ Posterior teeth in general
♦ Far more durable compared to composite fillings.
♦ There is almost no edge leakage.
♦ Using inlay-onlay fillings allows treatments without abrading the healthy tissues of teeth for crowns.
♦ The mastication surface and interdental parts of a porcelain filling are perfectly polished and therefore prevent bacterial growth and spotting.
♦As they are designed and prepared in laboratory environment, porcelain fillings almost perfectly match the anatomy of missing contours and contact points of a tooth. So they prevent the interdental accumulation of food residue.
♦As the mastication surfaces of porcelain fillings are prepared in a way to match the anatomic mastication line of the tooth, the mastication and grinding functions of porcelain fillings are far superior to composite ones.
♦As porcelain fillings establish smooth and pressure-free contact with the tooth, they prevent cracks and fractures that may occur in weaker parts of the tooth.
♦They preserve their aesthetic properties and durability for long years.
♦ The most significant disadvantage of porcelain fillings is that they are more expensive than composite fillings. The cost of porcelain inlay/onlay fillings is approximately 3-4 times more than the cost of composite fillings.
♦ While all other composite restorations can be completed in one single session, inlay/onlay fillings require more time such as two or three sessions depending on laboratory works. However it is now possible to complete the treatment in one single session with certain CAD/CAM system devices that are used by some clinics.
♦ Porcelain inlay/onlay fillings require more precision compared to composite fillings. If they are not prepared with required attention and precision, the result may be a failure.
♦The tooth is cleared off all decayed areas.
♦ Correction of “concave/undercut” areas and those lines that may hinder reliable measurement and preparation of the porcelain by using suitable filling materials.
♦ Precise measurement of the tooth to be crowned with porcelain and the transfer of the data to the laboratory.
♦ Temporary loading of the tooth to be crowned with a suitable filling material.
♦ Testing of the porcelain onlay-inlay that anatomically matches the missing tissue of the tooth.
♦ The porcelain filling must be in complete compliance with the tooth. If no problems are observed, the porcelain is cemented to the tooth by using an adhesive system during the same session.
♦Finalization of treatment.
